呼吸内科1997年成为大连市一级医学重点学科,1998年辽宁省医学重点专科,2007年辽宁省教育厅医学重点学科。1998年国家食品药品监督管理局批准为抗菌药物研究基地,2005年国家食品药品监督管理局认定具有药物临床试验资格研究室。2003年获得大连市卫生系统先进集体,2003年获得大连市卫生系统抗击SARS先进集,2006年科室获大连医科大学先进集体等体荣誉称号。
科室共有医生13人,其中教授7人,副教授3人,其中硕士以上学历5人,博士、博士后2人,博士在读3人。护士13人,其中副主任护师1人,主管护师1人,护师6人。主管技师1人,技师1人。
呼吸内科设有呼吸病房,呼吸重症监护病室(RICU)、呼吸内科专科门诊、变态反应门诊、变态反应检测室、肺功能检查室、纤维支气管镜室、睡眠呼吸障碍检查治疗室。科室配备进口呼吸机、监护仪、多导睡眠仪等设备,能高水平、高质量地完成多种呼吸系疾病的诊治工作。
呼吸内科在医疗技术、科研特色重点以下几方面:
1.抗感染方面
自1993年起主持了50余项抗生素临床研究,在国内有较好的声誉,参与了国内大多著名新型抗生素临床研究。临床研究从单纯临床观察进入分子生物学、基因水平研究,从耐甲氧西林金葡菌(MRSA)Mec、Fem基因研究、超广谱酶(ESBL)酶谱分析等;细菌防耐药变异浓度研究;动物实验抗菌药物连续滴注的药代学/药效学;应用协同试验和扫描电镜技术研究磷霉素与其他抗菌药物联合的体外抗菌活性;降钙素原(PCT)辅助诊断感染性疾病不同,使抗感染研究,在辽宁省内处于先进水平,纤维支气管镜防污染技术(PSB)采集下呼吸道分泌物诊断病原菌获大连市新技术奖。从而在抗感染方面经验丰富,在辽宁省内处于先进水平,并得到同行专业人士的认同。
2.重症呼吸衰竭抢救
是辽宁省内较早开展呼吸重症监护的单位之一,率先在大连地区开展重症呼吸衰竭机械通气治疗,成立呼吸重症监护病室(RICU),完善监护室监护抢救治疗设备,培养业务熟练、技术过硬的医护队伍,形成独特完善的监护管理措施,承担着内科和外科的危重病人的抢救工作,成功抢救许多危重病人,获得良好声誉。
3.支气管哮喘和过敏性疾病的过敏原检查和脱敏的治疗
开展支气管哮喘和过敏性疾病的过敏原检查和脱敏治疗,变态反应门诊应用国际上先进的皮肤点刺试剂和脱敏治疗试剂进行特异性过敏原检查和脱敏的治疗,并可抽血检查特异性IgE。安全可靠、敏感性高、特异性强、操作简单快捷,被检查者无明显痛苦,且疗效可靠。
4.睡眠呼吸障碍疾病研究
呼吸科在大连市较早开展了应用多导睡眠仪诊断鼾症等疾病,并应用无创通气进行nCPAP治疗。目前正在开展睡眠呼吸障碍疾病的基础研究。
5. 肺部肿瘤方面研究
化疗是肺癌的主要治疗方法,由于耐药性严重,降低了临床疗效,因此研究重点是肺癌的化疗耐药性。糖调节蛋白(GRPs)在肺癌耐药方面具有重要作用,课题组对GRPs在肺癌的生长、分化和化疗耐药等方面中的作用进行了系列研究,获得两项国家自然基金、一项辽宁省自然科学基金和辽宁省教育厅基金的资助,并分别获得辽宁省科学进步三等奖和大连市科学进步一等奖各一项。目前正与中国科学院大连化学物理研究所合作,应用微流控芯片这一具有高通量和集成化的优势的新技术,动态实时检测肺癌化疗耐药的过程,并与临床常用的传统研究方法进行对照,进一步探讨微流控芯片平台细胞效应性研究的优越性及应用前景。
呼吸内科近年来在完成各项医疗任务、提高医疗水平的同时,还致力于科研工作的开展和研究。共获科研立项13,其中国家级 3 项,省部级 6 项,市级 4项;各级各类科技奖励6项,其中省部级3项,市级3项。批准新技术立项5 项。出版专著3部,发表论文90余篇,其中国内核心期刊40篇,国家级期刊45篇,SCI收录 7篇。
Introduction of Respiratory Department
The respiratory department in 1997 became the Dalian medicine key discipline, in 1998 Liaoning Province medicine key faculty, in 2007 Liaoning Province education department medicine key discipline. In 1998 we are authorized for the antibiotics to study the base by National Food Drugs Surveillance Administrative bureau for the antibiotics to study the base, and in 2005 National Food Drugs Surveillance Administrative bureau recognized our department as the medicine clinical test qualifications laboratory. In 2003 we obtained the Dalian hygiene system advanced group and advanced group of the Dalian hygiene system to resist the SARS, and in 2006 the department attained Dalian Medical University advanced group and other collective honor awards.
There are 13 physicians of the department, including 7 professors, 3 associate professors. Among them, 5 persons are master, 1 MD, 1 postdoctor, and 3 physicians are reading doctors . There are 13 nurses, including 1 deputy director nurse practitioner, 1 nurse-in-charge and 6 nurse practitioners. There are 1 technologist-in-charge and 1 technician.
The respiratory department is equipped with respiratory disease ward, respiratory intensive care unit (RICU), the outpatient service of respiratory disease, the allergy outpatient service, the allergy examination room, the pulmonary function testing room, the fiber bronchoscope room, and the sleep-disordered breathing inspection and treatment room. The equipment of the department include imported respirators, ECG monitors, polysomnography and many other equipment, and the diagnosis and treatment of many kinds of respiratory diseases can be maded on a high level.
Characteristics of the respiratory department on medical technique and scientific research as following:
1. Anti-infection
We have managed more than 50 terms of antibiotic clinical research since 1993, and participated in the domestic many famous new antibiotic clinical research, obtaining fairly good reputation in domestic. The clinical research has been improved to the molecular biology and gene level research from simple clinical observation, research of Mec, Fem gene of methicillin-resistant Staphylococcus aureus (MRSA), analysis of the enzyme spectrum of extended-spectrum beta-lactamase (ESBL) and so on; research of variant concentration against drug-resistance of bacteria; the pharmacokinetics/pharmacodynamics of antibiotics continuous infusion in animal experiment; activity of anti-bacteria in vitro of combination of phosphonomycin and other antibiotics by co-using of experiments and scanning electron microscope; auxiliary diagnosis of different infectious disease by procalcitonin (PCT); All the research mentioned above raise the anti-infection research to the advanced level in Liaoning Province, and the protected specimen brush (PSB) of fiber bronchoscope gathering lower respiratory tract secretion to find out pathogenic bacteria won the Dalian new technology award. Thus we have rich experiences on anti-infection, and is on the advanced level in Liaoning Province, obtaining the professional peers’ approval.
2. Severe respiratory failure rescue
One of the earlier department of developing RICU in Liaoning Province, the earlist department to carry out the machinery ventilation treatment of severe respiratory failure in the Dalian area, establishing respiratory intensive care unit(RICU), improved emergency treatment equipment of RICU, training skilled business and perfect technique health care workers to form a unique sound RICU, bearing the rescue of severe physical and surgical patients, successfully rescuing many severe patients and obtaining good reputation.
3. Anaphylactogen inspection and desensitization treatment of bronchial asthma and allergic diseases
To carry out anaphylactogen inspection and desensitization treatment of bronchial asthma and allergic diseases. To carry out specific allergen inspection and the desensitization therapy by application of internationally advanced skin prick reagents and desensitization therapy reagents in outpatient clinic, and specific IgE can be tested. Safe and reliable, high sensitivity and specificity, fast and easy, no checks are suffering, and effect are reliable.
4. Sleep-disordered breathing disease research
Application polysomnography to diagnosis of diseases such as snoring earlier in Dalian, and application of non-invasive ventilation for the treatment of nCPAP. Sleep-disordered breathing diseases in basic research is ongoing.
5. Lung tumor research
Chemotherapy is the main treatment for lung cancer, and because of serious drug resistance, clinical efficacy is reduced. Thus our study focused on lung cancer chemotherapy drug resistance. Glucose-regulated protein (GRPs) in the lung cancer drug resistance plays an important role, so our research were carried out on the group of GRPs in the growth of lung cancer, chemotherapy-resistant and differentiation, accessing to the two National Natural Science Fundation of China, one in Liaoning Province Natural Science Foundation and Liaoning Province Department of Education Fund, and won third prize of scientific progress in Liaoning Province and the first prize of a scientific progress in Dalian. Currently working with the Dalian Institute of Chemical Physics, Chinese Academy of Sciences, applying the microfluidic chip technology with high-throughput and integrated advantages for dynamic real-time detection of lung cancer chemotherapy resistance in the process, in contrast with clinical research methods commonly used in tradition to further explore the microfluidic chip platform for cellular effects of the superiority of the research and application prospects.
Respiratory department in recent years to complete the task of health care, raise the level of medical care at the same time, is also committed to research work and research. A total of 13 scientific research projects, of which 3 on national level, 6 on provincial and ministerial level, and 4 on city level; winning 6 diverse kinds of science and technology awards at all, in which 3 on provincial and ministerial level, 3 on city level. Five new technology projects were approved. The number of publication of monographs are 3, and the papers published are more than 90 articles, of which 40 articles are published in the domestic Core Journal, 45 periodicals at the national level, SCI recorded 7.
